Providers must document in medical records: (1) prior therapy failures with aripiprazole AND at least one other antipsychotic (risperidone, olanzapine, quetiapine, ziprasidone, or lurasidone), (2) previous medication non-adherence that led to treatment failures, and (3) specific diagnosis (schizophrenia, schizoaffective disorder, bipolar I disorder, or major depressive disorder). For depression cases, document that Abilify MyCite is adjunctive therapy, not monotherapy. Claims will be denied without proper prior authorization.
