Summary
Humana Medicaid Ohio is implementing a new prior authorization policy for Columvi (glofitamab-gxbm), a cancer treatment for relapsed/refractory large B-cell lymphoma. Update EMR templates to document required criteria: diagnosis of DLBCL or LBCL arising from follicular lymphoma, two or more prior systemic therapies, and monotherapy use. Add prior auth alerts to ordering systems. Claims will be denied without proper authorization.
