Immediately: For Transaction Insight Portal users selecting Non-Person Entity (2), ensure Provider First Name field is left completely blank to prevent attachment linking errors and claim denials. By October 1, 2021: Emergency transportation providers (Type 06) can begin billing for ET3 services following requirements in AMPM 310-BB. Update documentation submission procedures to match UB-04 and CMS 1500 requirements listed in policy.
