Summary

Indiana Medicaid's Family Planning Eligibility Program has corrected diagnosis code coverage: B37.31 (acute candidiasis) is now properly covered and previously denied claims will be automatically reprocessed, while B37.32 (chronic candidiasis) and N76.82 (Fournier disease) are no longer covered. Claims with B37.31 from October 1, 2024 onward will be mass adjusted starting July 22, 2026.

Monitor remittance advices starting July 22, 2026 for automatic adjustments on B37.31 claims with ICNs beginning with 52 or 80. Review and resubmit any denied claims with B37.31 for dates of service after October 1, 2024 if not automatically adjusted.
