Action Required
By December 31, 2026: All providers in Medicare or Medicaid networks must complete the 15-minute HIDE-SNP & D-SNP Model of Care training. Choose Option 1 (individual virtual training with automatic attestation) or Option 2 (bulk attestation using the Priority Health MOC roster Excel template sent to DSNPtraining@priorityhealth.com). Ensure correct provider NPI numbers are used during registration or attestation to avoid 'incomplete' status. Late submissions will not be accepted.
