Summary
Effective May 18, 2026, Highmark will no longer accept paper claims submitted on outdated versions of the 1500 or UB-04 forms, and will return non-compliant forms to providers. Only original red NUCC-issued 1500 (02/12) forms and current UB-04 forms will be accepted for paper submission.
Action Required
By May 18, 2026: Billing team must ensure all paper claims use only original red NUCC-issued 1500 (02/12) Health Insurance Claim forms for professional claims and current UB-04 forms for facility claims. Remove any outdated claim forms from inventory and verify all paper claims have complete, valid, and readable information before submission. Consider transitioning to electronic claim submission through Availity Essentials or EDI to avoid delays and reduce costs.
