Summary
Nevada Medicaid has updated dental procedure codes with correct Area of the Oral Cavity (AOTOC) and Tooth Anatomy requirements, and fixed error codes 5064 and 5065. Claims missing required AOTOC/Tooth Anatomy information will be denied, and previously processed claims will be automatically reprocessed.

Immediately: Dental billing team must ensure all Nevada Medicaid dental procedure code claims include required Area of the Oral Cavity (AOTOC) and Tooth Anatomy information per ADA requirements. Review the ADA Dental Claim Data Recommendation document to identify which procedure codes require this information. Monitor future remittance advice for automatically reprocessed claims with error codes 5064 and 5065. Claims without required information will be denied.
