Nevada providers: Complete any pending enrollment applications before your group's blackout period begins. Group 1 (APRNs, Nurse Anesthetists, Midwives, PAs): Complete by April 13, 2025. Group 2 (MDs/DOs): Complete by May 4, 2025. Group 3 (Behavioral Health): Complete by June 8, 2025. Group 4 (All others): Complete by June 29, 2025. New enrollments after blackout periods must use Provider Flex tool.
