Summary

Nevada Medicaid is discontinuing its web-based Serious Occurrence Report (SOR) form effective April 7, 2025. All affected providers must now submit SORs through the new Therap system as 'General Event Reports' (GER) instead of using the previous NMO-3430A form.

Action needed
Immediately: All affected provider types (30, 38, 39, 48, 55, 57, 58, 59, 63, 83, 86, 95) must create an account with Therap system by contacting nvsupport@therapservices.net. Stop using the web-based SOR form and NMO-3430A form. Begin submitting all Serious Occurrence Reports as General Event Reports (GER) through Therap for Fee-for-Service or Waiver recipients. Failure to transition will prevent required SOR/GER reporting.
