Summary
Nevada Medicaid is reminding providers that incomplete documentation on DMEPOS orders will result in claim denials. All pages of multi-page records must include recipient's name, date of service, and page number.
Action Required
Immediately: Providers must ensure all pages of DMEPOS orders include recipient's name, date of service, and page number. Update EMR templates and order forms to include these required fields on every page. Review Web Announcement 3609 and MSM Chapter 100 section 103.13. Incomplete documentation will result in Medicaid claim denials.
