Nevada Medicaid is implementing new shadow billing requirements for Federally Qualified Health Centers (FQHCs) starting January 2026. Medical, behavioral health, and dental encounters must include both encounter codes and corresponding shadow procedure codes on the same claim, or claims will be denied with error code 6500.

By January 1, 2026: FQHC billing teams must update billing systems to require shadow billing - medical encounters (G0466, G0467, G0468) must include qualified medical procedure codes, behavioral health encounters (G0469, G0470) must include qualified behavioral health codes, and dental encounters must use new code D2999 with qualified dental procedure codes. Replace code 41899 with D2999 for all dental claims. Review the complete list of approved shadow billing codes on the Nevada Medicaid FQHC webpage. Claims missing required shadow codes will be denied with error code 6500.
