Individual providers must now enroll through agencies or Financial Management Services vendors to provide these services.

Nevada providers currently enrolled individually as PT 48 (HCBS Waiver for Frail Elderly) or PT 58 (HCBS Waiver for Physical Disabilities): Contact agencies or Financial Management Services vendors immediately to re-enroll through group enrollment. Update credentialing records to reflect new enrollment pathway. Failure to re-enroll through an agency will prevent billing for homemaker, respite, chore, adult companion, and attendant care services.
