Summary
Nevada Medicaid has released updated Presumptive Eligibility (PE) application forms (2990-EM and 2990-EM/S) effective January 1, 2026, which include CareSource as a new Managed Care Organization option. All previous versions of PE applications must be immediately destroyed and replaced with the new version dated 01/26.

Download updated forms from the Division of Social Services website DWSS-Applications & Forms. Ensure all staff use only the new application forms for PE determinations.
