Nevada Medicaid has automatically reprocessed claims that incorrectly denied with error code 6511 (Add-on code billed without primary) for dates of service from January 1, 2026 through February 18, 2026. Results of reprocessed claims will appear on the March 13, 2026 remittance advice.
Action Required
By March 13, 2026: Billing team must review remittance advice dated March 13, 2026 for results of automatically reprocessed claims that previously denied with error code 6511. Check for any new denials that may have occurred during reprocessing and initiate appeals if necessary according to Medicaid Services Manual Chapter 100.
