Summary
Nevada Medicaid has updated Form FA-30 (Out-of-State Placement Packet) to change terminology from 'Nursing Facilities' to 'Long Term Care Facilities' and updated contact information and tracking processes. The new form dated 04/16/2026 must be used immediately, and starting May 1, 2026, requests submitted on old versions will be returned.
Action Required
Immediately: Providers (types 11, 13, 16, 19, 44, 51, 68, 75, and 78) must download the updated Form FA-30 and FA-30-I dated 04/16/2026 from the Nevada Medicaid Provider Portal Forms webpage and discontinue use of previous versions. Before May 1, 2026: Ensure all out-of-state long-term care facility placement requests use only the new form version, as older versions will be returned for resubmission after this date.
