Summary
KMAP identified a system error where manually priced claims were incorrectly adjusted to $0.00 instead of being suspended for proper manual pricing, causing underpayments for claims processed between April 2022 and March 2026. Providers have 30 days to request adjustments for affected claims.

By October 29, 2025: Billing team must review all manually priced claims processed between April 4, 2022 and March 10, 2026 to identify potential underpayments. Contact KMAP Customer Service at 1-800-933-6593 with reference number CHG0072848 to request claim adjustments or resubmit affected claims within 30 days. Note that claims beyond 24 months old will require provider-initiated reprocessing.
