Immediately: Billing team must verify all provider information matches Medicaid records using the dom-azure-app.medicaid.ms.gov Provider Lookup Tool. Ensure NPI, taxonomy code, 5-digit zip code, and zip+4 on all claims exactly match provider file. For duplicate provider records, either deactivate unused Medicaid IDs or update zip+4 codes to create unique matches. Submit changes via MESA Provider Portal or fax to (866) 644-6148. Claims with mismatched information will be denied with Code 1347.
