For Alaska Medicaid patients: Within 5 business days of any transportation request, billing team must ensure proper medical justification is submitted including Letter of Medical Necessity for initial appointments, Plan of Care/Treatment Plan for ongoing care, and Medical Exhaust Letter when bypassing closer providers. Providers must document why telehealth cannot be utilized before requesting transportation. Submit documentation via fax to Conduent (907.644.5982), ANTHC (907.729.2403), TCC (907.459.3927), or email to tmomedicaid@anthc.org or PriorAuth@ykhc.org. Failure to submit proper documentation within timeframe will result in request denial.
