Summary
Alaska Medicaid implemented new attachment indicator requirements effective July 1, 2023, requiring specific indicators on claim forms (UB-04 field 30, CMS-1500 field 19, ADA 2012 field 35) or claims will be denied regardless of whether attachments are sent. The fiscal agent transition also updated travel authorization processes and claims handling procedures.
Action Required
By July 1, 2023: Billing team must update all claim submission procedures to include attachment indicators in correct fields - UB-04 field 30, CMS-1500 field 19 (not 9b), and ADA 2012 field 35 when submitting attachments. Update billing software validation rules and staff training. Claims requiring attachments without proper indicators will be denied. Ensure paper claims use original red forms only and electronic claims with attachments are faxed same day to 907-644-8122 or 907-644-8123 with proper cover sheet.
