By July 1, 2024: Stop including NDC information when billing B4158 claims to Alaska Medicaid. If asked for NDC information after July 1, forward the request to doh.hcs.medicaid.operations@alaska.gov. By October 1, 2024: Switch to using the new CMN forms for all DME authorization requests - update forms inventory and train staff on new requirements.
