Alaska Medicaid introduces new prior authorization requirements for intravenous immunoglobulin codes and specific Epoxia drug combinations effective May 11, 2026. Additionally, dental procedure code D9248 is no longer covered as of January 1, 2026, replaced by codes D9244 and D9245.
Action Required
By May 11, 2026: Billing team must update system to require prior authorization for HCPCS codes J1459, J1552, J1554, J1556, J1557, J1559, J1561, J1568, J1569, J1572, J1576, J1575, J1566, J1599, J1551, J1555, J1558 and J3490 with NDC 25357-0024-01. Use Physician Administered Drug Prior Authorization form. Immediately: Stop billing D9248 for dental services and use replacement codes D9244 and D9245 based on service rendered.
