Immediately: ICF/IID providers must establish process to notify New Mexico MAD/ESPB within 24 hours of all client discharges via fax (505) 827-3138 or secure email. Use MAD 378 form with Section A Box 1 marked 'Discharge' for community discharges or 'Transfer' for DDW transfers. Only complete Sections A and B. Track LOC abstract submissions to stay within 10 late submissions per calendar year limit to avoid retroactive review denials.
