Summary

Starting December 1, 2025, all prior authorization requests for high power prescription lenses (7.25+ diopters) and polycarbonate lenses for adults 21+ must be submitted through the Comagine Health Provider Portal (CHPP) for New Mexico Medicaid Fee-for-Service. Direct emails to Medical Assistance Division staff will no longer be accepted.

By December 1, 2025: Vision care providers must register for the Comagine Health Provider Portal (CHPP) at https://comagine.org/program/new-mexico-medicaid/providers to submit prior authorization requests for high power prescription lenses (7.25+ diopters) and polycarbonate lenses for adults 21+. Only one registration per company location is needed, with 7 business days processing time. Stop sending direct emails to Medical Assistance Division staff for these authorizations after the effective date.
