Summary
Alabama Medicaid is eliminating the Out-of-State Hospital Update Form effective May 1, 2026. All hospital providers will now receive 5-year enrollment periods but will be purged if they don't have paid claims within 24 months. Currently active out-of-state hospitals must complete revalidation by the end of their current 6-month extension.

Failure to revalidate will result in enrollment termination.
