Summary
Out-of-state, non-cost reporting hospitals in Illinois can no longer use state-generated procedure code 0090 for services not on the Ambulatory Procedures Listing (APL). These hospitals must now enroll for a separate provider number using their FEIN plus a 3-digit identifier to bill fee-for-service claims on professional claim format.
Action Required
Before October 16, 2003: Out-of-state, non-cost reporting hospitals must contact the Illinois Provider Participation Unit at 217-782-0538 to enroll for a new fee-for-service provider number using their FEIN plus 3-digit identifier. Stop using procedure code 0090 for services not on the APL and switch to professional claim format with new provider number for these services.
