Action Required

Action needed
By February 1, 2004: Medical providers in the specified Illinois counties must obtain prior approval from First Transit (877-725-0569) for all non-emergency transportation. Only medical providers can arrange standing orders for renal dialysis, behavioral health, chemotherapy, radiation therapy, and physical therapy by faxing completed Standing Order Request forms to (312) 327-3855 at least two business days prior to trips. Update staff procedures to ensure compliance as claims without proper authorization may be denied.
