Non-emergency transportation billing changed origin and destination codes from ten to four codes effective April 15, 2004. Providers must use different crosswalk tables and billing procedures based on whether their approval letters were posted before or after April 15, 2004.

Immediately: Billing team must determine approval letter posting dates and use appropriate crosswalk tables (Table 1 for pre-April 15, 2004 approvals, Table 2 for post-April 15, 2004 approvals). For paper claims, use codes from approval letters without HCPCS modifiers. For NSF electronic claims, crosswalk to two-digit numeric values in FAO Record Fields 10.0 and 11.0. For future HIPAA 837P claims, convert to HCPCS modifier codes. Do not contact First Transit to change codes on pre-April 15 approvals.
