Summary

By December 1, 2004: Transportation providers must update billing systems to use HCPCS modifiers (P for Physician's Office, D for Medical Service, H for Hospital, R/K for Residence, L for Other) instead of origin/destination codes A, B, D, K, E, N, S, X. Use the modifier shown on First Transit approval letters to avoid claim rejections. Share modifier conversion table with software vendors and IT departments.
