Summary
The Illinois Supportive Living Facility (SLF) handbook has been updated with rule revisions and clarified reporting requirements. Form DPA 2448 (Physician Certification) requirement has been deleted from the handbook but must still be submitted until the Department completes system programming changes.
Action Required
Continue to complete and submit Form DPA 2448 (Physician Certification) to the local Department of Human Services office for all Medicaid residents in Supportive Living Facilities until the Illinois Department notifies SLFs that programming has been completed.
