Illinois Medicaid updated covered renal dialysis injectable drug codes effective January 1, 2006. New codes J1751, J1752, and J0882 are now reimbursable, while old codes J1750 and Q4054 are obsolete and no longer covered.
Action Required
Immediately: Billing team must update system to use new HCPCS codes J1751 (Iron Dextran 165), J1752 (Iron Dextran 267), and J0882 (Aranesp) for renal dialysis claims. Stop billing obsolete codes J1750 and Q4054 as they will be denied. Verify current rates and maximum dosages from Illinois fee schedule at hfs.illinois.gov/feeschedule/. Update UB-92 forms for category of service 25 claims.
