Summary
Effective July 20, 2006, all durable medical equipment (DME) providers must include the prior approval number on claims for DME and supplies. Claims missing this number or with incorrect numbers will be rejected with error code R09.
Action Required
Immediately: DME billing team must update all claim submission processes to include prior approval numbers. For paper claims (DPA 2210), enter the last 8 digits of the prior approval number in the Prior Approval Number field. For electronic claims (837P), enter the prior approval number in Loop 2300 REF02 at claim level, and Loop 2400 at service line level if different. Update billing software to require this field or claims will reject with R09 error code.
