Summary

Low-Level Laser Therapy (LLLT) now requires prior authorization and is only considered medically necessary for prevention of oral mucositis in cancer patients using FDA-approved devices. All other uses are considered investigational and not covered.

Before January 1, 2026: Billing team must implement prior authorization requirements for all Low-Level Laser Therapy procedures. Update system to flag LLLT claims requiring documentation of FDA-approved device, cancer treatment status, and oral mucositis prevention indication. Prepare to collect required documentation: History and Physical/Chart Notes, current symptomology, manufacturer and model name of device, and documentation of cancer treatment causing high risk of oral mucositis. Claims without proper authorization will be denied.
