Summary
Illinois Cares Rx members who failed to reapply before January 1, 2007 have lost prescription coverage. Medicare members will lose wrap benefits but retain Medicare Part D coverage, while non-Medicare members have no prescription coverage until they reapply using Form IL-1363.

Front desk staff should verify Illinois Cares Rx eligibility for all members before filling prescriptions. Direct members who lost coverage to reapply using Form IL-1363 at www.cbrx.il.gov or www.illinoiscaresrx.com, or call Senior HelpLine at 1-800-252-8966. Online applications will be available February 5, 2007.
