Summary
Illinois Department of Human Services will begin conducting targeted reviews of nursing facility residents with serious mental illness through Mental Health PASRR providers. This will initially affect facilities in 8 counties, with statewide implementation beginning July 1, 2007, and includes new Targeted Case Management services for residents transitioning to community settings.
Action Required
Immediately: Nursing facilities in Cook, Lake, Will, McHenry, Kane, Kendall, Grundy, and Kankakee counties must prepare to provide Mental Health PASRR agencies with timely access to identified residents with SMI, their medical records, and treatment team staff.
