Summary
Long term care facilities in Illinois must report resident deaths within 5 working days using Form HFS 1156 via fax to DHS, unless they participate in the REV System which allows electronic submission. Facilities with high overpayment rates due to late notifications will be required to join the REV System.
Action Required
Within 5 working days of resident death: Long term care facilities must fax Form HFS 1156 to DHS at 217-557-4210 and maintain fax confirmation sheets. REV System participants must submit electronically through their vendor instead. Send original Form HFS 1156 to local DHS caseworker within 5 working days for non-REV facilities.
