Summary
Illinois Medicaid eliminated prior authorization requirements for polycarbonate lenses for children through age 20, effective August 17, 2007. Prior approval is still required for patients 21 and older.

Immediately: Update billing workflows to stop requesting prior authorization for polycarbonate lenses for patients age 20 and under when billing Illinois Medicaid. Continue following prior approval requirements from July 14, 2006 notice for patients age 21 and older. Review billing instructions in the Handbook for Providers of Optometric Services, Appendix 1.
