Illinois Medicaid changed its preferred insulin products to Novolog/Novolin (short-acting) and Levemir (long-acting). Prior authorization will be required for non-preferred insulin products (Humalog/Humulin starting December 17, 2007, and Lantus starting January 7, 2008).

By January 7, 2008: Add prior authorization requirement for Lantus insulin. Providers should proactively switch patients to preferred insulin products (Novolog, Novolin, Levemir) when clinically appropriate.
