Summary

Effective May 1, 2008, hospitals and institutional providers must transition from UB-92 to UB-04 claim forms, begin reporting National Drug Code (NDC) for certain outpatient drugs, and include Present on Admission (POA) indicators for all diagnosis codes on inpatient claims. UB-92 claims will be returned after the effective date.

By May 1, 2008: Billing teams at hospitals, hospice agencies, renal dialysis facilities, and ambulatory surgical centers must update all claim submission systems to use UB-04 forms instead of UB-92. Register as UB-04 subscriber at nubc.org. Configure systems to report NDC codes in FL 43 for renal dialysis drugs (including Epogen Q4081) and expensive add-on payment drugs. Program POA indicators in FL 67 for all diagnosis codes on inpatient claims. UB-92 claims submitted after May 1, 2008 will be returned, and claims without required NDC codes after July 1, 2008 will lose add-on payments.
