Summary
Illinois Medicaid updated covered injectable drug codes for renal dialysis effective April 1, 2008. The new HCPCS code Q4098 (Iron Dextran, 50 mg) is now covered, while codes J1751 and J1752 (Iron Dextran 165 and 267) are obsolete and no longer reimbursable. All injectable drugs require NDC reporting on outpatient renal dialysis claims.
