By October 1, 2008: Billing team must update all UB-04, 837I, and DDE claim submissions to include NPIs for attending physicians (all claims except outpatient renal dialysis), operating physicians (inpatient and surgical outpatient claims), and other physicians (outpatient dialysis and rehab). Update billing system to prevent submission of claims without required NPIs to avoid rejections with error codes H43, H44, and H45.
