Summary
Illinois HFS and All Kids medical cardholders must select a Primary Care Provider and Health Plan for their medical home, with different managed care options available by county. Most cardholders statewide must enroll in the Primary Care Case Management (PCCM) program, while certain counties offer additional Managed Care Organization (MCO) options.

Contact Illinois Client Enrollment Broker Provider Helpline at 1-877-912-8880 for Cook, Adams, Brown, Henry, Jackson, Madison, Mercer, Perry, Pike, Randolph, Rock Island, Scott, St. Clair, Washington and Williamson counties, or Illinois Health Connect Provider Helpline at 1-877-912-1999 for all other counties with enrollment questions.
