Summary

Effective August 6, 2009, Lantus® will replace Levemir® as the preferred basal insulin for Illinois Medicaid patients. Patients currently on Levemir® can continue through October 31, 2009, after which prior authorization will be required for medical necessity.

By August 6, 2009: Begin prescribing Lantus® instead of Levemir® for new basal insulin patients on Illinois Medicaid. For existing Levemir® patients who cannot switch to Lantus®, providers must submit prior authorization requests by calling 1-800-252-8942 or faxing 217-524-0404/217-524-7264 before October 31, 2009. Update prescribing protocols and notify providers of the formulary change.
