Summary
Hospice agencies must now use Pay-To Provider NPI numbers instead of one-digit payee codes in Form Locator 2 on UB-04 claims. The transition period ended October 1, 2010, and claims with old payee codes will no longer be accepted.

Immediately: Hospice billing teams must update UB-04 claim forms to use Pay-To Provider NPI numbers in Form Locator 2 instead of one-digit payee codes. Download updated Appendix K-2 from the Illinois Department website and replace the October 2008 version in your billing manual. Claims submitted with old payee codes will be rejected.
