The approval notice form HFS 3076F is discontinued and First Transit now generates approval notices directly. A new fax number has been established for the prior approval agent.

Transportation providers only: Update contact information for First Transit Inc. to use new fax number 630-873-1450 for standing prior approval requests. Replace handbook pages HFS T-211 (1-4) with updated versions. This does not affect medical billing teams unless they provide transportation services.
