Action Required
By May 13, 2011: LEAs must complete and submit electronic cost calculation forms through HFS MEDI system for each type of direct medical service provided during 2009-2010 school year. Obtain Illinois digital identity to access MEDI via myHFS website. Complete all required form sections (Lines A, B, C, G, I, M) with FTE staff numbers, hours worked, salaries/benefits, and indirect costs. Failure to submit by deadline will prevent claim adjustments and block future claim processing.
