Summary
Illinois is transitioning approximately 40,000 Medicaid clients from the Your Healthcare Plus disease management program to a new Integrated Care Program starting May 1, 2011, affecting six counties. The remaining 250,000 members will lose Your Healthcare Plus benefits on June 30, 2011, and transition to Illinois Health Connect for primary care services.
Action Required
By May 1, 2011: Billing team must identify Medicaid patients in DuPage, Kane, Kankakee, Lake, Will and Suburban Cook counties enrolled in Your Healthcare Plus and verify their transition to the Integrated Care Program for disease management services. By July 1, 2011: Update patient records for remaining Your Healthcare Plus members to reflect transition to Illinois Health Connect program.
