Immediately: All hospital staff must report any third-party requests for Medicaid patient information to the Illinois Department of Healthcare and Family Services Bureau of Collections. Fax requests to 312-793-8169 (Cook County) or 217-524-5672 (other counties). Include patient name, DOB, recipient ID number, service dates, and requesting entity information. Cannot refund Medicaid payments to assert own liens on personal injury settlements.
