Summary

Action Required
Within 60 days of identifying any Medicaid overpayment: Billing team must prepare a complete Disclosure Report including provider information, detailed claims list in secure Excel/Access format, and investigation summary. Submit by mail to Illinois Department of Healthcare and Family Services Office of Inspector General. Failure to report within 60 days subjects claims to False Claims Act penalties ($5,500-$11,000 per claim plus triple damages).
