Action Required

Action needed
Immediately: Ambulance providers must enter ALL non-emergency hospital discharge transports into First Transit's PassPORT system, even without MCA forms. Contact hospitals immediately when MCA forms are missing and request completion. Hospital billing teams must ensure MCA forms are completed and transmitted to First Transit and ambulance providers BEFORE ordering transports. Contact First Transit at 1-877-725-0569 for questions or hfs.mcaform@illinois.gov for MCA form issues.
