Summary
Oversight of the Colbert Consent Decree Implementation has transferred from Healthcare and Family Services to the Department on Aging. Clinical staff from Lutheran Social Services and Metropolitan Family Services will conduct reassessments on select Colbert Class Members with serious mental illness in Cook County nursing facilities.
Action Required
Facility staff should facilitate access to nursing facility residents and their medical records when LSSI and MFS clinical staff arrive for reassessments and Resident Reviews. Staff will be notified in advance of visits and should verify identification upon arrival.
